As a result of recent client surveys, we’ve expanded our Custom Forms Module to enable clients to evaluate all employees and support staff throughout their districts.
Upload additional support staff. To begin using this new feature, districts will need to purchase additional licenses, upload their additional staff, and add the appropriate roles.

The new and improved Custom Forms module has been expanded to include the following:
More question types and easier means of rearranging questions and sections. Simply click the rearrange option to easily move sections throughout the form.

Handy calculation total that tallies form total score. Multiple choice questions are given a numeric value which calculates a total final score.

View Usage Reports. Easily download user data for a particular form for further review.
